This is a Mamiya 645 Pro 150MM F4 C Telephoto Lens.  this lens will work on all Mamiya 645 Cameras including the 645 Autofocus Cameras.

This lens is also in excellent condition with no haze, no fungus, and no separation.   Comes with the built-in lens hood, both front and rear caps, as well as the lens case. Lens has "San Antonio Police Lab" etched into the upper barrel but it does not effect the operation of the lens at all.
